Position Summary: Under the direct supervision of the Coordinator, the Aegis Senior Care Group, LLC is responsible for assisting individuals in accordance with the Individual Plan (IP) for the purpose of developing and improving basic life skills, fostering self-reliance, and independent living. Key Responsibilities: You will assist clients with daily living including: • Personal Care. • Help with Daily Activities. • Assistance with ambulation, transfers and other safety assurance. • Establishes a therapeutic relationship with the individual receiving services to ensure effective guidance, support and service delivery. • Provides support to the individual to ensure objectives are met in accordance with the Service Plan. • Provides transportation for individuals receiving services to planned and/or necessary activities and appointments. • Attends training and other meetings as required. • Maintains confidentiality, respects human rights and practices universal precautions in accordance with Aegis Senior Care Group, LLC policy. • Meets with the Coordinator at least monthly for clinical supervision, or more often if indicated.
