<![CDATA[Background: <br> The Center for Advocacy for the Rights and Interests of the Elderly (CARIE) is a non-profit organization based in Philadelphia, dedicated to improving the quality of life of older adults. CARIE was founded in 1977, primarily as an advocacy organization for the frail elderly. Its initial program, the “CARIE LINE,” quickly became recognized throughout the region as a resource for a wide range of individuals and institutions seeking to resolve elder care issues. Today, the agency's telephone consultation service is just one aspect of a coordinated array of programs. <br> <br> Position: <br> This full time position reports to CARIE’s Ombudsman Coordinator. The Long Term Care Ombudsman Program (LTCO) is under contract with the Philadelphia Corporation for Aging. This program covers 20 nursing homes, 67 licensed personal care homes, 75 Domiciliary Care Homes and 13 Older Adult Daily Living Centers in North Central, Center City, West, and South Philadelphia. Ombudsman is a Swedish word meaning "citizen representative." The long-term care ombudsman serves as an advocate for long term care consumers by handling complaints and educating residents and families about their rights. <br> <br> Duties and Responsibilities: <br> <br> • Investigate and resolve complaints made by and on behalf of residents in long term care facilities <br> • Respond to calls providing information and advocacy about issues regarding long term care consumers <br> • Attend and participate in resident/participant meetings and family meetings in long term care settings <br> • Make regular visits to long term care facilities to observe facilities, staff/consumer interaction, check for safety, cleanliness, etc. <br> • Respond to requests for assistance and in-services for staff <br> • Analyze public policy affecting long-term care services and assist in developing comments <br> • Provide information about long term care issues to the community through speaking engagements, on-going involvement with other organizations, hearings, etc. <br> • Represent CARIE and the Ombudsman Program on committees and workgroups <br> • Attend CARIE staff meetings and committees as assigned, attend regional ombudsman meetings and required trainings <br> • Write occasional reports, articles, or other materials <br> • Review literature (reports, articles, studies, etc.) on a regular basis to stay current on long term care issues <br> • Maintain documentation in ombudsman database <br> • Other related duties as assigned <br> <br> Educational/Experience/Skill Requirements: <br> <br> • Bachelors’ degree in Social Work or related field <br> • Motivation to advocate on behalf of long term care consumers and a willingness to learn <br> • Experience in and/or knowledge of aging policy, resources for older persons and their caregivers, and the long term care system <br> • Excellent verbal and written communication skills and knowledge of Microsoft Office <br> • Demonstrated competency in direct, as well as indirect, practice <br> • Willingness to attend mandatory training outside of Philadelphia <br> • Willingness to travel throughout the city of Philadelphia using public transportation and flexibility to work occasional evening and weekend hours <br> • Ability to remain objective and impartial while investigating complaints <br> • Public speaking/training expertise <br> <br> Required Screening: <br> <br> • Criminal Background Check <br> • Professional References <br> <br> Physical and Mental Demands: <br> <br> • Must work well under pressure and have the ability to be creative and analytical. <br> • Must be able to use public transportation to travel to North Central, Center City, West, and South Philadelphia. <br> • Must be observant and able to attend to details of environment while visiting nursing homes, personal care homes, domiciliary care homes and adult day centers. <br> • Must be able to walk up stairs. <br> • Must be able to operate office equipment including but not limited to computer, telephone, fax, copier; move freely around the office; sit or stand at a desk and telephone for long periods of time; communicate in person, and over the telephone, with individuals and groups. <br> • This position will require occasional work outside of normal office hours. <br> • Some lifting of 10-20 pounds may be necessary. <br> <br> Ombudsman must be free of conflict of interest and cannot be employed by, have an ownership interest in, or be involved in the management of a long term care facility while employed as a long-term care ombudsman. <br> ]]>

<![CDATA[<h3>Summary</h3> <p>The Grassroots Advocacy Manager reports to the Vice President for Public Policy and works closely with the Government Relations Manager and others to plan and implement grassroots advocacy initiatives to advance public policy positions of the Cultural Alliance and its member institutions. The Grassroots Advocacy Manager will have primary responsibility for the Cultural Alliance's online advocacy initiatives, building and engaging the list of supporters, writing advocacy alerts and other supporting materials, and researching issues which have an impact on the Cultural Alliance's mission and public policy priorities. The Grassroots Advocacy Manager will also develop strategies for and implement other advocacy activities, such as public events and rallies and connecting members with their elected officials.</p> <h3>Primary Responsibilities</h3> <ul> <li>Manage the Cultural Alliance's Online Advocacy Center. Prepare electronic Advocacy Alerts, identifying issues, targets and calls to action. Generate reports on responses to campaigns and provide customer service to advocates.</li> <li>Develop and implement strategies to grow the list of Advocacy Alert subscribers. Utilize social media tools such as blogs, Web forums, and social networking websites to reach new audiences and engage them in the Cultural Alliance's advocacy initiatives.</li> <li>Research and report on government activity on issues of importance to the Cultural Alliance, staying abreast of current developments. Write newsletter articles, website content, and reports to funders and board committees on these issues and Cultural Alliance activity regarding them.</li> <li>Support member institutions' advocacy efforts. Provide information on contacting government officials, including sometimes making contact on their behalf. Provide materials and advice on engaging elected officials in the work of our members. Where appropriate, implement advocacy campaigns on their behalf using Cultural Alliance e-advocacy system.</li> <li>Work with Web & Database Task Force to ensure that grassroots advocacy needs will be met by new Cultural Alliance website and constituent relationship management database. Work with Operations Manager, Web & Publications Manager, and vendors as needed to ensure tight integration between e-advocacy tools, website and database.</li> <li>Participate in Cultural Policy department meetings. <![CDATA[Basic Function: Work with multidisciplinary team to identify and engage MSM clients, with the goal of entering treatment for substance abuse and/or behavioral heath issues. Assist clients thereafter in maintaining treatment, making appointments and meeting goals. <br> <br> Specific Responsibilities: <br> <br> 1. Successfully complete Peer Specialist certification. <br> 2. Conduct outreach activities targeting men who have sex with men (MSM), employing the Social Networks model, among others. <br> 3. Engage 250 MSM with substance use/abuse messages. <br> 4. Recruit 50 MSM for pre-treatment activities. <br> 5. Document and monitor progress and participation of clients connected to services. This includes the completion of six-month follow-up GPRA surveys – and complete online data entry – for clients recruited into outpatient substance abuse treatment. <br> 6. Facilitate referral to formal substance abuse assessment with Mazzoni Center Assessment Specialist. <br> 7. Facilitate linkage to HIV testing and STD screening <br> 8. Participate in ActionAIDS staff and supervisory meetings, as well as regular team meetings with Mazzoni Center staff and other team members. <br> 9. Participate in training, supervision and staff development as required. <br> 10. Maintain accurate records and documentation of services provided, including submission of demographic and statistical reports of outreach, recruitment and pre-treatment activities to the Program Manager on or before the 5th of the month. <br> 11. Solid written and oral communication skills are necessary, with proven documentation and reporting skills. <![CDATA[GENERAL DEFINITION <br> <br> The Career Coach is the primary contact for clients. The Career Coach is responsible for the facilitation of the one-week orientation program for their clients to prepare them for their entrance into their transitional work and/or community service experience. Will guide the clients thorough the transitional work experience and support the client during the client’s employment placement for the first six months from the date client’s date of hire. The Career Coach will provide comprehensive case management services that support the client throughout the duration of the program. <br> <br> The work includes facilitating a one week orientation for the clients to ensure that they have their supportive services in place, an updated resume and necessary tools to start employment. Conduct weekly one-on-one meetings with clients to provide feedback and guidance to the individual. Maintaining case management files on individual clients and providing management with appropriate reports on client’s status when requested. (subjected to BETP Guidelines) <br> o Review, complete and submit client’s timesheet to MIS and Finance as scheduled. <br> o When necessary, identify intensive barriers to employment and refer clients to appropriate social service provider. All such referrals will be documented in the client’s Employment Development and Retention Plan (EDRP), and client progress will be tracked and documented. <br> o Update all termination files by the 5th of each month for the previous month’s terminations. <br> • Facilitate the Transitional and Unsubsidized Work Experience: <br> o Assist the Work Partner in providing guidance and support to the client during the transitional experience, i.e. coaching, troubleshooting time and attendance issues and work behaviors. <br> o Meet with clients regularly to support the client in their work. <br> o Conduct two site visits per month and complete monthly progress (MPR) report for all clients on caseload. <br> o Distribute, review, and complete time sheets for the Clients to record their hours while employed by TWC and explain the TWC pay policies and procedures. <br> o Ensure that clients receive the appropriate client incentives offered by TWC by collecting the necessary documentation at each benchmark represented in the contract. <br> o Conduct and document periodic assessments of client progress weekly. <br> o Build relationships with a wide variety of Philadelphia’s social service agencies and ensure that TWC clients can access those services when needed. <br> • Assist in the Placement of Clients Into Unsubsidized Employment: <br> o Coordinate and participate in meetings between the client and Sales staff. <br> o Prepare client for interviews. <br> o Ensure client resume is complete. <br> • Enter data into the following databases: ANEMONE and CWDS on a daily and/or weekly basis.
